Choose the form that matches the behavior or skill you want to track, then define the target behavior clearly before collecting data. Print the forms for paper use, or open the editable PDF in Adobe Acrobat Reader to use the fillable and automatic features.

Start With a Clear Target

Use the Student Behavior Profile and Target Behavior Definition to record the student's strengths, concerns, supports and the specific behavior you plan to observe.

Before starting or changing an intervention, the Baseline Behavior Tracker can be used to collect 3–5 days of information.

Choose the Right Tracking Form

Select a form based on what you need to measure:

  • Frequency forms for how often a behavior happens
  • Duration forms for how long it lasts
  • On-task and attention forms for focus during lessons
  • Participation and task completion forms for classroom work
  • ABC and anecdotal forms for observation notes
  • Trigger, intervention and self-regulation forms for supports and responses

Review Patterns and Progress

Use the Behavior Pattern Analysis and progress forms to look across the information you collected. The Individual Behavior Goal, Weekly Progress Monitoring Graph, Behavior Progress Summary and Student Support Meeting Summary can help organize information for team discussions and review meetings.

Adaptations

  • Use only the forms that match the student's current behavior goal instead of completing the full set.
  • Use printed forms on a clipboard when collecting data while moving around the classroom.
  • Use the editable PDF when you want typed notes, automatic calculations, clickable boxes or the self-plotting progress graph.
  • Use the summary forms to organize information when several teachers or support team members contribute observations.
