Start with the Quick-Start Guide and choose the behavior or area you want to track. Select the form that matches the information you need, then type into the editable PDF or print a copy for handwriting.
Start With a Clear Target Behavior
Use the Student Behavior Profile and Target Behavior Definition to describe strengths, concerns, current supports and the behavior you want to observe.
If you are beginning or changing an intervention, the Baseline Behavior Tracker can be used to collect data for 3–5 days first.
Choose the Right Tracking Form
Select a form based on what you need to measure:
- Frequency and daily frequency
- Duration and weekly duration
- On-task behavior and attention
- Participation and task completion
- Self-regulation and classroom behavior
- ABC observations and anecdotal notes
Review Patterns and Progress
Use the Behavior Pattern Analysis and Trigger & Support Tracker to look for patterns in the information you collected. Continue with the Behavior Intervention Tracker, Individual Behavior Goal, Weekly Progress Monitoring Graph or progress summary forms when appropriate.
Adaptations
- Use only one tracking form at a time when you need a simple observation system.
- Print the forms for staff who prefer recording data by hand.
- Type directly into the editable PDF when you want a clean digital record.
- Choose different forms for classroom observations, intervention monitoring, IEP data or student support meetings.

Are the forms editable?
Yes. The PDF forms allow you to type into the lines, table boxes and writing boxes. Text shrinks to fit when needed.
What sizes are included?
The resource includes US Letter and A4 PDF versions. Each PDF is 29 pages and includes the cover, 25 editable forms, page numbers, terms of use and a thank-you page.
Do the PDF forms calculate data automatically?
Yes. In Adobe Acrobat Reader, supported forms include automatic totals, averages and percents. The weekly progress graph can also plot dots from the numbers you type.
What formats are included?
The resource includes printable and editable PDF files, Microsoft Excel and Google Sheets formats. The Excel version includes formulas and a chart.
