Materials and preparation

  • Printed copies or a computer or tablet for the editable files
  • Adobe Acrobat Reader for editable PDF features
  • Microsoft Excel or Google Sheets if using those formats
  • Pen or pencil if completing the forms on paper

Choose the target behavior or area you want to monitor and select the form that matches your purpose. For baseline tracking, collect data for 3–5 days before starting or changing an intervention. Print the forms you need or open the editable version on your device.

Choose the Right Tracker

Start with the question you want the data to answer.

  • Use frequency forms to count how often a behavior occurs.
  • Use duration forms to record how long it lasts.
  • Use the On-Task Behavior Tracker for interval observations.
  • Use the ABC Behavior Observation to record what happened before, during and after a behavior.
  • Use the intervention and goal forms to document supports and progress.

Record Clear, Objective Data

Describe what you can see and hear instead of making judgments about the student's intentions. Use the checkboxes, rating scales, tables and writing areas to keep observations consistent and easy to review.

In Adobe Acrobat Reader, selected forms include automatic totals, averages and percents. The weekly progress graph plots dots from the numbers entered, while the paper version can be completed by drawing and connecting the dots.

Review Patterns and Plan Next Steps

Use the Behavior Pattern Analysis, Trigger & Support Tracker and progress forms to look across several observations. Before a meeting, the Behavior Progress Summary and Student Support Meeting Summary can help organize goals, data, decisions and action steps.

Adaptations

  • Use only one tracker at first when the team is focusing on a single behavior.
  • Shorten the observation period when a full lesson or school-day observation is not practical.
  • Use printed forms for handwritten observations or the editable files for typed records.
  • Pair adult data collection with the Feelings Check-In or Behavior Reflection when student input is appropriate.
