Materials and preparation

  • Printed forms and a pen or pencil for paper use.
  • A computer or device for typing in the editable PDF.
  • Adobe Acrobat Reader for PDF calendars, automatic calculations, checkboxes, and graph features.

Choose the behavior or area you want to observe and decide which form matches the information you need. Print the pages you plan to use, or open the editable PDF, Excel file, or Google Sheets version before beginning.

Start With a Clear Target

Use the Student Behavior Profile and Target Behavior Definition to describe the student's strengths, concerns, supports, and the specific behavior you want to observe.

When appropriate, use the Baseline Behavior Tracker for 3–5 days before starting or changing an intervention.

Choose the Right Tracking Form

Select a form based on what you need to measure:

  • Frequency forms for how often a behavior happens.
  • Duration forms for how long it lasts.
  • On-task, attention, participation, or task completion forms for classroom or learning behaviors.
  • ABC Observation and Anecdotal Observation Notes for understanding what happens before, during, and after a behavior.
  • Trigger, pattern, self-regulation, and intervention forms for reviewing supports and responses.

Review Progress

Use the Weekly Progress Monitoring Graph, Behavior Progress Summary, Individual Behavior Goal, and Student Support Meeting Summary to organize information over time.

The Parent Communication Log can help keep a consistent record of family contact.

  • Track one clearly defined behavior at a time when a simpler starting point is needed.
  • Use printed forms for quick handwritten observations or editable files for typed records.
