Start with a clear behavior
Use the Student Behavior Profile and Target Behavior Definition to describe the student’s strengths, concerns, supports, and the exact behavior you want to observe.
When appropriate, use the Baseline Behavior Tracker for 3–5 days before starting or changing an intervention.
Collect the right kind of data
Choose forms based on what you need to know:
- Frequency forms for how often a behavior happens
- Duration forms for how long it lasts
- On-Task, Attention, Participation, or Task Completion Trackers for learning behaviors
- ABC Behavior Observation and Anecdotal Observation Notes for what happens around the behavior
- Self-Regulation and Trigger & Support Trackers for strategies, situations, and helpful supports
Review progress
Use the Behavior Pattern Analysis and Behavior Intervention Tracker to look across observations. Record measurable goals with the Individual Behavior Goal form, follow weekly data with the Progress Monitoring Graph, and summarize information before a student support meeting.
